Not everyone has the ability to work and with the way things look, there won't be a social safety net left. We need to start looking at the big picture.

Instead of small unions being on their own, small unions could create a co-op model so they would have negotiating power with insurance companies. This still ties healthcare to employment. Not everyone has the ability to work and with the way things look, there won't be a social safety net left.
