5️⃣ identify funding or logistical (e.g. virtual) opportunities to address reported barriers to participation (e.g. travel costs & distance).

1️⃣ logistics: consider challenges & solutions with multi-centre studies (e.g., how to integrate different clinical processes & care across centres).

2️⃣ eligibility: including an upper limit for the length of time since the patient has been listed for HTx

6. 👟 n=2 patients in PREHAB: HIIT was safe as evidenced by no exercise-related adverse events and patients were able to sustain increases in mean peak power output (PPO) and heart rate (HR) from the first to last exercise bouts (see Figure).

5. 🔎 Why so few patients?! Being transplanted, medication and device contraindications, trial commitment, transportation barriers, and HIIT not recommended by medical professionals (see detailed CONSORT).

4. 📅 Trial recruitment began in October 2018 and closed, due to the COVID-19 pandemic, in October 2020. Of the 84 patients screened, 17 were eligible and consented to participate; 2 patients were randomized to each intervention group.

3. 🏥 Study design: A multi-centre RCT with a target sample size of 60: n=30 > HIIT, n=30 > usual care (as per Canadian Cardiovascular Society guidelines).

2. 🥅 Main goal of trial: We evaluated the impact of a 12-week program of PREHAB (i.e., high-intensity interval training [HIIT], stress management class, and nutritional workshop) compared with usual care in patients with advanced HF awaiting heart transplantation (HTx) on functional capacity.
