We did our best to address this issue in the study design. All patients started empiric antibiotics on hospital day 0-2. "Delayed" meant escalation to broad-spectrum on day 3 or later. Delayed were matched to early broad-spectrum by hospital, admitting dx, and a composite of 28 clinical variables.
