The point is that teams the Bills and Ravens were in the early season are not the same teams as they are now.
As much as the dull analysis is the Ravens thumped the Bills 35-10, it was week 4. And yet the Bills ended up second seed and the Ravens weren't.

Or it could be the players didn't recognise the potential for the fake. According to Joe Marino the Broncos did their homework and saw that Ingram has a tendency to back off, giving the opposing player space.
The Broncos saw the chance and exploited it.
Maybe it was good old lack of execution.
