that's oversimplifying it -- FAA investigations really do a great job of making sure every single causal factor is accounted for
it comes across as blaming everyone/nobody because failures in complex systems are always a cascade not a single point of failure

the only honest internal investigations i've ever seen were IT root cause analyses and that was only when the culprit was outside IT itself
