After a long process, our article — written with @mbabeenchama.bsky.social and a formidable team incl @daniellaparra.bsky.social — has just been published

It explores how racism operates as an invisible but powerful determinant of health in Spain. ojs.sanidad.gob.es/index.php/re...

What do we talk about when we talk about ... conditionalities? "in 2021, Sanofi received $415 million in federal funds to build a flu vaccine–manufacturing plant in Canada. This deal could perhaps have included a requirement to apply for approval of rifapentine with Health Canada."

And it’s not only middle and lower-income countries. In Europe, lenacapavir is being sold for Tx at absurdly high prices, completely detached from both national GNI (capacity to pay) and actual R&D costs.

Over a quarter of all new HIV acquisitions happen in the 26 countries and territories that Gilead has chosen to leave out of its license. That means a large number of people will be unable to access affordable prices for this lifesaving prevention.

Together, these reforms show how national law can give real-world shape to abstract global commitments—WHA resolutions, WTO declarations, OECD guidance—and turn them into enforceable, effective policy for global justice. /10

Working on Spain’s draft Medicines Law with @adrianaruiz.bsky.social , we asked:
How can national legislation bridge local governance and global health justice?
Here’s what that kind of legal translation could look like 👇 /3

🔹 Before WHA78, China’s voluntary funding amounted to just 17 cents for every dollar of its assessed contribution — a modest 0.17 to 1 ratio.
🔹 After WHA78, that ratio surged to 4.5 to 1 — China now gives over four times its obligatory contribution.

In this view, color represents actor type (governments, ngos, academia, private). The position of each actor reflects how they are connected: those who collaborated more frequently are pulled closer together./5

In the map:
🎨 Color = community/cluster (e.g. themes or alliances)
🔵 Size = strategic bridging role
🔠 Label size = number of direct connections

Some actors are tightly embedded, others sit at the periphery.
Some are lone connectors between silos./3

🌍 Who connects the dots in global health governance at #WHA78?

Dozens of side events in Geneva brought together states, UN agencies, philanthropies, activists, and others. But who served as the bridges across all that noise?

Here’s a visual story. 👇 🧵1/

📊 Who's showing up big at WHA78?

China’s delegation is 6x larger than expected for its income level. France, Germany, Japan & Spain also stand out.

TRIPS flexibilities are a critical, legitimate, and legal tool for ensuring access to medicines. It should be noted that countries of all income levels have used or considered using them, not just the World Majority. /1

Debt cancellation isn’t a new idea — it’s been used across history, from ancient Babylon to post-WWII Europe.

In moments of crisis, societies recognized that crushing debt held back recovery and justice.

Why not use a tool we've always had? Debt relief must be part of the solution.

Today, $4 out of every $10 in government revenue in low- and middle-income countries goes to debt interest payments — only $1 goes to health.

46 countries spent more on debt than on health in 2022, affecting 3.3 billion people.
April 27, 2025 at 8:08 AM
In high-TB burden countries, 43% are in or near debt distress.
80% of funding for TB programs usually comes from domestic sources — but debt servicing is draining these funds.

Result? Less TB prevention, diagnosis, and treatment.
