Reading "Where is the Evidence for Evidence Based?" by Shelder, I feel this astonishment again, this time accompanied by anger. I believe research is an awesome part of science, and the abuse of these tenants is so frustrating and disappointing.

What if the therapist is concerned that the patient is going downhill, or showing narcissistic or self destructive tendencies? Reading these notes would have a completely different effect on a patient. If therapy notes are accessible to the patient, the notes are at risk to become performative.

As Hoffman says, reading the therapy notes give Mr. Baldwin a sense of pride. He is proud of his progress and reading the notes helped him come to the realization that he had improved. However, this is a situation where the patient is getting better and the therapist writes complimentary things.
