Some thoughts… a quarter got cross over, they randomized people to a site that did not believe in steroid benefit so the never used it, and lastly compare the APACHIE scores to cape cod/ the req for pressers makes this more a sepsis study rather than a CAP study.

Two things. One is we know that septic shock patients have low intrinsic vasopressin upon admission. Second is that they initiated vasopressin at doses much higher than we would in practice. At about 0.25mcg/kg/min of norepi, vasopressin and stress dose steroids are being added in general here.

6-12 months unless the patient has recurrent infections of the same thing (ie UTI).

Our institution uses it a lot. Not sure of its efficacy tbh. No true renal cut offs but it does have a preservative that can can accumulate in very poor renal function if given IV chronically.
