IA Medicaid outsourced to private insurers. Payment for ER visits reduced if dx was not on list created by companies. List would include things like fracture of left ankle, but not right ankle. State would deny appeal of claim. Hospitals had to work with state to update list.

I used to work for a small rural hospital in IA., in charge of admitting, insurance, and billing. Had multiple times where patient would present to Dr. office with symptoms that would warrant an immediate CT. With preauthorization taking 1-3 days, patients would be told to go to ER. No preauth in ER
