My understanding is that indie bookshop have very limited shelves and storage, so they do stuff on lower volume (so the rebate is not as good).
But this may apply only to France (from where most of my reference come from). "Exemplaire edition" offer such a pack (translation in alt). Hope this help.

The rule I'm aware of is "Top to bottom" then "Left to right" in case of equality.
I'll agree it's best to have both, but in this case the compromise is acceptable due to the panel and page composition.
I was able to find some other examples in my comic collection.

The 0.5% is an upper figure. My guess for books is 1/3rd of that.

Note: the English wiki page "History of comics" is very short and lacking images. Look at what's on the French one for 1880! There's a part discussing tech improvement and its impact on comics, the main tech allowing cheap comics is from 1870
