I found that a teeny bit of sedation and music (patient's request), works best.

I find myself enjoying the experience, especially when they sing along .. and they amazed that "it was not what they thought it would be".

Yes! I work here way up here, in the mountains. I don't know how hospital classifications go there, but here, we're a level 2 hospital. As with other 3rd world countries, funding for public hospitals is difficult.

So, we make do with what we can 😁

Wow, discussions like this is an eye opener. In our setting, we have 4 OT but only 2 capnographs. As with other techs, those things are a bit pricey here, hence most of our GA cases don't use one. Most of the time, we go clinically ..

In our setting, the emergency drugs (ephedrine, atropine, sux, etc.) are usually drawn up on the 1st case of the day and refilled accordingly as the cases go by. Each anes has their own tray / box for these.

Wonderful video Amit! 👏👏 Thanks for keeping RA alive and kicking!

Just a suggestion, maybe you can blend a video or pic (side by side maybe?) of an actual block being done?

So novices (like me 😁) can have a better see of where the needle should go?

We do hips via a posterolat approach .. did a 97 y/o recently but AS not that severe:

Bit of prop+ket (the one Jeff Gagsden does) ➡️

Fem OOP (I've grown to liking Ki Jinn Chin's approach) ➡️

turn the px to the side ➡️

Place a bit of QLB to cover the T12 - L1 dermatome ➡️

TCI low dose

I start with 2% Sevo and build up every two breaths by 2% to max 8%. If you have N2O you could (not should) use it to make things smoother.

DONT rush it.
Keep talking & reassuring

Nice! We do too! We also "prime" the mask with a fruity scent, usually with a marker. Sometimes we let the kids choose the marker. The citrus is a favorite for the adults. Someone once asked for a beer scent but I don't think they make those.

Same too! ENT case, thought the patient was awake, tried to deepen the anesthesia. Senior came in, gave me the eyebrow, and told me to read more on Halothane 😅
