Bhattcharya is right about doing like Sweden next pandemic, but he is dishonest about the reasons. The reason Sweden had the best outcome with the lowest amount of unnecessary deaths on the EU continent were the social policies. Never once does he mentions that. www.thelancet.com/journals/lan...

Great thread, and I really appreciate you engaging even with smaller accounts. My main concern is that the real lesson might be missed—that in a pandemic, maybe giving people clear information and letting them make informed decisions could be more effective than trying to force behavioral change.

Perhaps the reason similar voluntary measures seemed less effective in the UK summer 2020 was that UK began with a strict, enforced lockdown. This may have led to quicker lockdown fatigue—especially when combined with the arrival of summer.

I think the description of Sweden's restrictions as light because they were voluntary is false. If you educate the population so they understand the logic behind the measures, compliance becomes more effective and sustainable. I think this is well indicated by mobility data.

Das sind Gefühle, die sich nicht in den Daten widerspiegeln. Niedrigste Übersterblichkeit und die wenigsten verlorenen Lebensjahre auf dem Kontinent sprechen für sich. Welche ethische Ungerechtigkeit man Schweden auch vorwirft – anderswo war sie offensichtlich größer.

Palliativversorgung ist oft von Beginn an Teil der Versorgung. So alte und gebrechliche Menschen werden in keinem Land typischerweise intensivmedizinisch mit invasiven Maßnahmen behandelt.

Pflegeheime in Schweden unterscheiden sich stark von denen z. B. in Deutschland. Von 2 Mio. Menschen über 65 leben nur ca. 10.000 in solchen Einrichtungen. Die durchschnittliche Lebenserwartung nach dem Einzug beträgt etwa 7 Monate.

Etwa die Hälfte aller COVID-19-Todesfälle ereignete sich in Pflegeheimen – das galt für die meisten Länder, auch für Schweden. Der Anteil war vergleichbar mit dem der Nachbarländer; der Unterschied lag vor allem im Zeitpunkt der Ausbrüche.

Your concerns are worth considering, but they don’t undermine the study’s conclusions. The increase in non-COVID excess deaths is real, not just a data artifact. Overlooking the broader impacts of the pandemic would be a misstep.

Yes, national death register data would be great, but the study covers 18 countries. Focusing only on a few NSOs with better data would limit scope. The authors also use Monte Carlo simulations to address uncertainty.

You mention heatwaves & flu (which definitely played a role), but these don’t explain the persistent rise in non-COVID excess deaths over 3 years. Pandemic-related healthcare disruptions are still a strong candidate for part of the excess.

Even if COVID deaths were undercounted, that doesn’t explain why excess mortality kept increasing post-2021. The study looks at all-cause mortality, so the trends remain valid regardless of classification issues.

You raise some fair points, but the study explicitly accounts for potential undercounting of COVID deaths. It models two scenarios—one assuming all excess mortality was due to COVID, another assuming some deaths were misclassified. That uncertainty is built into the estimates.
