Is it non-capture, and the spikes are artifact of the pacing, not depolatisations? Also non-sensing if its in a demand mode. If there was capture previously, then maybe the pads need changing. Happily, the BP and O2 sats aren't that shoddy considering the pulse.

:-) ok, doesn't look like a pacing check, is it dislodgement of a lead?

Dont know, but the rate looks like 68 bpm which seems atypical for a PPM, and they are only visible in V1, and don't appear to be sensing or capturing appropriately so... not pacing spikes?
