I meticulously planned a route to do all the side quest areas in metaphor for one segment without having to go back to town, only to realize I didn’t go to the market area and missed a quest which would have been on my route.

Playing DnD with the homies. Go fight the big bad. I decided to do a little jig which failed so horribly the big bad was distracted. I cast polymorph, somehow the boss fails the save, turning boss into a cat. We dropped the boss from ~300 ft in the air and they died from fall damage.
