Also, your post kind of made me realise what exactly I've been feeling is off with Candela Obscura 🤣 I've realised that I'm not used to systems who are that mechanically complex (relatively) and have NO combat rules. I can do simple mechanics with no combat, or complicated with combat rules. 🤣

I was meaning to get into the system! I'm not opposed to taking away the 'traditional flow of combat' but is it more like YZE or PbtA? Or is it just plain narrative with a few rolls to enhance?

I wouldn't consider a filling as a surgery, more like a procedure? Removing wisdom tooth tooth though could be a surgery, depending if you go under for it or just get topical anesthetic 🤔 But it might be cultural difference, since I'm EU. It's not common in my country to use gas ect.
