He does think that. I'm sure the docs didn't want him to realize why they wanted to check, so they sugarcoated it. And it's probably true that Reagan, before him, refused it because he knew full well what it was for and what it might show. Biden may have refused it as well, and for the same reason.

MRIs are indeed used as tests for one's heart (see below) when monitoring/observing is recommended or required which means that there is, indeed, a serious medical concern to begin with. MRIs can be full body or from neck down. That would be a question I would ask. I suspect it was the former.
