Newer (Pro) iPhones have a LiDAR sensor that can measure the distance wich can improve the accuracy of the focussing.

Well you said it, there is not much for your phone to focus on. Focusing mostly works on comparing different image content. Now when there is a lot of blue sky there isn’t a lot of contrast or textures that help the focussing system. And a fast moving object doesn’t help either.

Does it happen only occasionally? Then try focusing „manually“ by tapping on the screen on your subject. If it happens repeatedly your camera may be broken.
