Spent a career in IPC. Now just wanting IPC guidance to acknowledge the evident mode of transmission for SARS-CoV-2 and other respiratory pathogens - airborne.
Without good guidance there is no person safety.
Doctor of Nursing.
Rapid infection control will only work if it is targeted on how transmission arises.
So off to find how transmission happens

It says either HCWs are not following guidance or the guidance is ineffective at preventing transmission and infections.... I am for the latter.

This is a single celled fungi. It has no ability to cause outbreaks - its not a general with complex battle plans ready to invade. IT IS US who provide the perfect niches - the ICU is an agar plate promotes transmission and enables proliferation on invasive devices and various liquids...

Taking the data from a systematic review and using a Pareto.
It can be seen preventing Bloodstream infections is C. auris control. I see not national/international guideline that includes preventing BSI.
Secondly it is also airborne distributed and we are not recommending air cleaning either...

Can we stop with the Semmelweis nonsense.
He should have copied the midwives and not taken his students to the PM room (every day).
The recommendation for a disinfectant akin - that burned the skin - was never a sustainable solution.

The Truly Courageous Blog accepts it will take truly courageous steps to admit errors. But the health of so many is dependent on these courageous steps.

The population, specifically HCWs, cannot afford to be made more sick because decision makers cannot admit their errors.

IPC have to answer 'How' questions with mechanistic evidence (ME) to use RCTs would be inappropriate.

Many things in IPC were discovered by ME
How surgical infections happened
The best ventilation for theatres
How pathogens spread.....

IPC guidance considers high-risk = AGPs
The highest risk is to people caring for infectious, symptomatic people in close range.

For goodness sake give the most exposed RPE.

Admit AGPs did not & do not turn droplet or sprayed on transmission to mucous membranes into airborne (inhaled infections)

The whole of 2 NIPCMs (England & Scotland) is written based on this nonsense. Droplets unless AGPs is make-believe.

IPC guidance needs answers to HOW questions before intervention effectiveness Qs, i.e., HOW TRANSMISSION HAPPENS

1) How does MRSA, or SARS2, or CDI transmit?

The answer will/must/can only involve mechanistic evidence.

This is why IPC guidance was & is wrong

This is a serious thread.
@UKHSA say they have robust systems for avian influenza prevention. However something is awry. It appears that in Europe (mainland), the US and Canada, airborne transmission is accepted. But not in the UK. First the CDC "or is inhaled"

No No No. Semmelweis did not propose "simple hand washing". His procedure burned the skin.
Stop blaming the HCWs for not following him - it was akin to paint stripper & would NEVER be allowed today

So in chapter 3.... Standard Precautions...
I have separated out the environment must have / consist of... and just put the HCWs actions for the basics of safety for all to prevent HCAIs, not just reduce the risk from recognised and unrecognised sources....
