Fwiw, I just finished it again tonight - it's 'the returned cultist case' quest! I went to the crossroads after the quest completed then back to the lighthouse and got the conversation/unlock.

You shouldn't be locked out! I did the same and still got hero status. I think you need to speak with her in the lighthouse after her quest to unlock it.
