resolving shoulder dystocias. Sure, it’s a legitimate concern to be watchful of, but it’s not a fear that I personally carry as a midwife. Subsequently, I feel that’s why I have rarely seen them in nearly 1000 births. I’m patient. Less than 5 that I can recall…and all babies were healthy and fine.

No, I absolutely believe they did. It’s a much-feared complication, but it’s often (not always) caused by the provider. It’s fascinating to me how much fear and trauma some birthworkers bring into the birth room. I see midwives pull on babies when they’re scared—exactly like what OB’s do.

What he may not understand (or doesn’t care to anyway), is that this increases the maternal mortality and morbidity of birthing people. It’s also another attack on bodily autonomy if unnecessary cesareans start trending again.

A contracted pelvis, however, is not resolvable. Those babies will only come out “healthy” with surgery. So, my concern about Musk’s somewhat dimwitted statement is that he might be too interested in increasing the cesarean rate for specific populations to force some type of twisted evolution.

Shoulder dystocias are actually fairly uncommon amongst well-trained out-of-hospital midwives. I’ve seen more vaginal breech births than concerning shoulder dystocias in my career. The Zavanelli Maneuver is rare for a reason—shoulder dystocias are typically resolvable.
