The review is likely to recommend more gatekeeping. Formally, it will inform a new service specification for the NHS, which then may eventually impact private provision via the CQC.

In practice, GP prescription refusals are already common

It had enabled her life to feel “ordinary”. Suddenly her prescription had been stopped. The message explained that the GP practice was unable to “safely support ongoing prescribing or monitoring” of the “specialist drug”. If Emily wanted a new prescription, she would have to go private – a cost she could not afford. One thing to certainly expect as the result from the Levy review is a separate under-25 service: as recommended by Cass, recent NHS documents state that currently "in the process of a tender" for a pilot

Unlike the multi-year Cass review, the Levy review is rushed. Announced August 2024 and initially expected by spring.

Documents from various NHS trusts show that they expect it to be released soon, but with the timelines slipping; first mid-June, July, and still not out yet

NHS England adult gender dysphoria services review, aka the Levy review, directly follows the Cass review that infamously examined youth services.

Cass explicitly recommends a service for under-25s; this and the neurodivergence concern are a common theme

Screenshot: Clinicians who spoke to me felt that, in common with the population that is presenting to children and young people’s services, the majority of patient presentations were extremely complex, with a mix of trauma, abuse, mental health diagnoses, past forensic history, ASD and ADHD, and therefore this limited assessment was inadequate. These other clinical issues were not addressed or taken into account in decisions to prescribe masculinising or feminising hormones.

Everyone's talking about the EHRC guidance banning trans people from single-sex spaces like toilets + changing rooms... but there is another looming threat in the Levy Review

The stated motivation for commissioning the review included concerns about "inadequate, time-limited assessment", weak consent procedures, waiting times, patient demographics, and administrative problems.[3] It is still underway. Though NHS England originally expected to publish the report in spring 2025[4], it has not yet been released as of yet. Internal documents suggest publication may be close. Its likely effects are uncertain, but it may lead to tighter gatekeeping in NHS services, with possible knock-on effects for private clinics, particularly for people under 25.

While the anti-trans project will still probably largely succeed, it is one area in which they overplayed with some potentially risky (to them) consequences. [...]
