I would normally not have batted an eye at this paper

But it’s by Robert Hahn, and is a nice overview of everything we know about fluid redistribution in the critically unwell- including gel phase, some glycocalyx stuff, 3 compartment models, and albumin escape rates

Where does the fluid go?
💦 morphological changes/damage by fluid overload
🛌 fluid distribution in anesthesia
3️⃣ extracellular fluid compartments
💧 physiology, manipulation & max filling of slow-exchange fluid space
🔥 inflammation, capillary leakage & lymphatic flow 💦

I have now treated 8 colleagues from my own hospital, including major emergencies like cardiac arrest. They ALL regretted how much they’d been working and resolved to reduce stress

The most recent said “I don’t want to be the hardest working doctor in the graveyard”

Been thinking about that one

I love love LOVE scanning the heart 😍, but when it comes to bang for your buck for ultrasound novices, lung ultrasound is such a winner 🏆

It's time for another #POCUS basics #skeetorial 🧵

Let's talk about common pitfalls in #LungUltrasound and how to correct these ✔️

point-of-care electrolyte analyzers cannot detect hemolysis!

so point-of-care hyperkalemia may or may not be real.

in this case, the hyperkalemia was wholly artifactual.

think twice before unleashing therapy for hyperkalemia (the ECG is often the key)

- I like to employ gentle compression with the probe at my intended site 📌

This reassures me that, even when I am compressing the soft tissues slightly with my freezing needle or catheter, my ascites pocket remains open.

I created a starting pack for #POCUS peeps! Help me by sharing it and suggesting more people to add! It also contains the two feeds I created, for #echofirst and #pocus. go.bsky.app/EtZL9J6
