At around 700 ilvl/50 traits, I do withotu too much difficulty every +16.
Exceptions are arcway - I think it's bugged, I managed to +3 a +17 and bosses lasted 50 seconds.
And maw of souls, that one is harder than the rest during to Helya's immense damage.
