Huge shift to direct-to-inject, protocols that use 8mg>24mg range weekly dose to initiate, with general gist of - lower doses less likely to cause BPOW (better if very recent use, BUT necessitates f/u in 1-3 days max), 24mg working for lots w COWS > 4 & gives more leeway to give monthly injection.

BHN a clinic system near me (with Dr. Ruth Potee) have this amazing new policy and calculator that is so reasonable (and so reassuring to people like me who were initially trained to not consider interim use in decision of how to adjust dose!)

We had something called “Centricity” in residency- it was a solid framework on top of which our MD-IT phenoms programmed amazing tricks like “.hepclabs” to pull all hep c labs into note and calculate fib4 for you. EPIC briefly in fellowship. Now Athena, which is ??? Ok. Lots of clicking.

I got used to our change! Eventually! I mean I didn’t know how to easily see phq2 results til 2 months ago, & today got confused about how to delete a mistakenly opened pregnancy episode (rather than “close” one) but it’s all fine, it’ll be great! 🙂🫣🙃
