The logical solution is train more doctors and to use carrots (+ the occasional stick). But this is expensive!

So the alternative, since the 1960s, has been workforce substitution. This is attractive, as it certainly is true

Just off the Leng Review call.

Once again it strikes me just how existential the issue of workforce substitution is and what is at stake.

The root cause is not enough doctors. Historically, not enough in rural and remote places. With the coming 'Silver Tsunami', not enough

partly on the RCP and I have written 2 books on the History of the College.

I ran for Council because the SLT wanted to sell off a big chunk of the Historical Library. I was told that despite being the Librarian, my views didn't count.

First as an Invited Services Reviewer in *2009*.

Have served almost continuously on various committees since then. Some not very glamorous.

After I finished, I stayed on to complete my Physicianly training at the John Radcliffe in Oxford.

I made choices out of expediency, given the rules of the Home Office and the RACP.

My CCT is in standalone GIM, which was pretty unusual for the time. But it is not a choice

I was accepted to do a DPhil at Oxford. Won several scholarships, including a Wellcome Trust Studentship.

While doing the DPhil, I locumed about the place, which gave me an insight into the state of the NHS in the early 2000s. Pretty awful, in comparison with home!

the History of Medicine at Cambridge.

Wow! This was a revelation!

Like, literally changed the course of my life.

I wrote my thesis on William Heberden the Elder. Which sent me down all sorts of other interesting trains of thought that continue to play out in my research

I did my house jobs at the Princess Alexandra Hospital. My bosses were (mostly) outstanding. And despite the business of their jobs, they were prepared to help me become a much better doctor.

My time at the University of Queensland was terrific.

I went through in the Golden Age of Australian Med Ed. The course was tough, but well constructed. It produced excellent clinicians.

I went to local schools and then to boarding school, like many rural kids.

I actually grew up wanting to be a lawyer. But was dissuaded from this by Maths teacher (thanks Mr Kerr!).

To get into Med School, I had to graduate in the top 1% of my school leaving year.

I come from rural Queensland. My mother is a community pharmacist and my father farmed and owned various small businesses.

My parents were big on honesty, decency, hard work (my mother thinks I am a lazybones!) and education.

They especially talk tough on reforming the GMC, which is seen as a BARRIER to their proposals.

Then there are the 'new roles'.

So not only will all HCWs be able to do doctors' jobs, but new jobs will be created to do their jobs too.

Distinctions between disciplines will dissolve, including having common stems for HCWs at uni and an increased ability to just swap between disciplines. 🤯

Strangely, at the same time as wanting more doctors and a consultant-delivered service, it is baldly stated that EVERY type of trained HCW will be able to function as a doctor.
