And the fragmentation of acute care, lack of continuity & less experienced staff lead to tendency to less confidence in recognising need to change track & prevent the next admission. Lots to do but all possible

Hi Helen. Really useful thread. It’s interesting that it’s something many of us have witnessed but having the data is really helping focus. More and more we see busy clinicians struggling to know when to stop doing complex tests and really focus on what will make a difference.
