Nothing unusual for me: posterior approach, cemented stem bypassing the holes. I would consider DM as well, if I have concerns about stability.

WIDE exposure. Incisions heal from side to side, not from top to bottom. I don’t want a fracture

Anterior capsulotomy. I find this one basically turns it into a regular THA. I create a pocket where to put the trunnion and keep it mobilized underneath a cobra lodged in the anterior column

No, we can’t see it, unfortunately. This is a real problem for surgical cases :(
