The European pilot was released in the UK before the show broadcast in the US. I remember being a little confused when the series pilot aired over here.

Only big-selling authors will get a say on a book cover. It's very much the publisher's remit and territory. New editions will get a new cover as design tastes evolve. Album art is often artist driven and part of the overall package - it's their baby. To change anything would be heresy.
