My kiddo went to the ER as an infant for swallowing something that got stuck in her esophagus. They did an XRay to figure out what it was before doing surgery to retrieve it.
Insurance denied the XRay as medically unnecessary until I appealed. Claimed it was an error it was denied.

I’ve got 3 diabetics in my family and all 3 have had their insulin refills randomly denied for no reason. My one conspiracy theory is that insurance randomly denies prescriptions in the hope that no one complains and they save money. If someone complains, just say it was a mistake and approve.
