It was a joke about if there was actually a doll based on the "with autism" phrasing that some news outlets use AND a joke about how cheap Mattel tends to be with the Fashionista dolls and their limited accessories.

So if Mattel had actually released a doll they called "Barbie With Autism", it probably wouldn't have been a collab with ASAN, and then that doll probably wouldn't have the wrist+elbow joints, so the production cost would be a bit lower, allowing for another plastic accessory. [3/4]

-used the correct language and called the doll Autistic Barbie because they collaborated with ASAN, and it was also ASAN that pressed them for the extra features like wrist+elbow articulation so the doll can stim. The other Fashionista dolls don't have these because it's more expensive. [2/?]

Except Mattel would be too cheap to let the hypothetical "Barbie With Autism" have the Plastic Concept of Autism on top of the accessories she already has (Barbie Fashionista dolls usually only get one plastic accessory, it's noteworthy that the real Autistic Barbie got three.) [3/3]

Sensory accessories aren't the abstract concept of "autism" though. The joke was that the phrasing "has/with autism" implies the abstract concept of "autism" as a separate accessory... and Barbie dolls have separate accessories. [1/?]

Wait nvrmind if Mattel did a 'Barbie With Autism', that would indicate that they didn't collab with ASAN, didn't get told to give her extra wrist+elbow articulation to stim, she probably wouldn't have the sensory aid headphones either, so Mattel COULD afford An Autism as a separate plastic accessory

Now I'm imagining a 'Barbie With Autism' that comes with An Autism as a separate plastic accessory. Oh wait nevermind, Mattel is too cheap to add another. They already gave Autistic Barbie THREE (fidget spinner, headphones, AAC tablet) instead of the standard one.
