3) It depends on the situation. Even in straight FFS systems like Medicare, specialist's often require referral

No. And purge, like Trump? No again. We choose Democratic politicians who are suited and best able to win in their state or District. That's how we become a big party and win. Not by blind ideological exclusion and purging! SAD.
