Ah well i think we all understand that trade off 😆

Somewhat surprised by the vitriol, he's not definitely wrong and he certainly doesn't say methods are unimportant. It seems pretty logical that if you are unimpressed (or don't understand) the results, the methods section would be useless (to you), no?

Don't let primary doctors and big pharma tell you not to smoke. It's fine if you just follow up with a Pulmonologist every month for pulmonary function testing and low-dose CT scans. 🫁

I don't think the average Arsenal amateur comentator is comprehending the magnitude of expense that a deal for Rodrygo would cost bsky.app/profile/dgol...
Madueke: £50m (fee) + £21m (£80k/wk x 5 years) = £71,000,000 (total cost)
Rodrygo: £90m (fee) + £50m (£190k/wk x 5 years) = £140,000,000 (total cost)
And this likely underestimates Rodrygo's cost as he may command £200k+/wk wages. #arsenal

Do you know if this generalized across infection location and source (ie. Pna v bacteremia v soft-tissue v osteomyelitis)?

Potential, not likely, and possibly not even clinically relevant

The decision to add a particular test would depend on that specific test. ICU patients have already tested quite a bit. The best way to save money on ICU patients is to get them out of the ICU faster.

"Precision medicine" is a very broad term, meaning nothing specific, so I am unsure what costs you're referring to, but I see negligible costs or any at all. If you're referring to oncology and genetic profiling, this isnt that. This is primarily referring to clinical data and hemodynamics.

I believe this transition will occur within my lifetime and its impact will be significantly more profound than the immune-oncology revolution (which...was also profound)

Interesting, I remember thinking the intercellular machinery for peptide processing involved in cross-presentation was fairly advanced and important in serving regulatory functions, but perhaps I misunderstood

I would imagine the overall result of option 1 being a chaotic immune response with some outright dangerous possibilities. It probably makes sense why the immune system did not evolve to permit this pathway to be readily accessible.

It's probably time to upgrade this hypothesis, as it looks more likely to be plausible and definitely supports the continued use of azithromycin in undifferentiated acute COPD exacerbations.

Because no one has ever really demonstrated to me how this was likely to occur, I had probably filed this away as "unlikely to be true" in my brain. These data suggest that AZM has direct effects on human type 1 immunity in a sterile model.
