Total-Contact Casts: Healing Ulcers Fast—with Remarkably Low Reported Thrombosis Risk #ActAgainstAmputation @ResearchatUSC @ALPSlimb #Offloading
While total-contact casts (TCCs) remain, along with other irremovable devices the gold standard for off-loading diabetic foot ulcers in people who can tolerate them, concerns about immobility and venous thrombosis have lingered. Our recent systematic review found surprisingly low rates of deep venous thrombosis (DVT) in patients treated with TCCs—only 1 in 136 cases (~0.7%), compared with rates up to…

SOLARIO: Redefining the Role of Systemic (and local) Antibiotics in Bone and Joint Infection #ActAgainstAmputation
For decades, prolonged courses of systemic antibiotics have been standard practice after surgery for orthopaedic infections—osteomyelitis, prosthetic joint infection, fracture-related infection, and yes, even diabetic foot infections. The reasoning was simple: biofilm is hard to reach, and more antibiotics must be better than less. But what if, with the right local antibiotic strategy, less really is more? The Study The…

In honor of Día de los Muertos, my deepest thanks to AMCICHAC — the Asociación Mexicana de Investigación en Cicatrización A.C. — for your warmth, wisdom, and hospitality during our time together in León. We can modify this for clinic and our patients to "muer-toes" 🙂 This beautiful Catrina, gifted by our colleagues, reminds us that life and death walk hand-in-hand — and that service, science, and compassion can help keep more people walking through life on their own two feet. Here’s to collaboration, community, and our shared goal of eliminating preventable amputations. AMCICHAC MÉXICO

Predicting Success in Spinal Cord Stimulation for the Diabetic Foot: The Role of Infrared Thermography
Spinal cord stimulation (SCS) has been an emerging therapy for patients with intractable ischemic pain and non-healing diabetic foot complications. While traditionally used for pain relief, evidence is building that SCS may also enhance blood flow, support ulcer healing, and even prolong limb salvage. But the critical question remains: who benefits most from this expensive and invasive therapy? A new study by…
