Epi is doing a data analysis, writing a paper in a week and then defending to a committee. Biostat is part stat in-class exam and part data analysis + report, then defending.

At ULaval, Epi and biostat students have a comprehensive exam at the end of their first year of PhD, then a protocol defence usually during the second year. They have to write a protocol in a format that closely resembles a grant proposal (CIHR or NSERC), evaluated by a committee and then presented.
