From the video CNN is showing, it is clear the driver of the vehicle was waiting and waved cars to go by, and even waved the truck containing the ICE people to go by before that truck stopped. The claim they were under threat is a lie. The driver was simply trying to get out of the way.

No medical examiner worth his or her salt (or medical degree) would make the nonsensical statements Trump attributes to his physicians. However, it is said familiarity with testing items can boost scores, potentially hiding early dementia, so maybe that's why they're having him take them so often.
