TRAIN compared Hgb <9 & <7 transfusion strategies in 850 patients w acute brain injuries (TBI, ICH, SAH). At 180 days liberal transfusion = better neuro outcomes (62.6% vs. 72.6% unfavorable) & fewer cerebral ischemic events (8.8% vs. 13.5%).

Also our belief that certain meds don’t decrease mental status. Fentanyl gtts one of the biggest culprits due to context-sensitive half-time. When can we have remifentanil in the ICU??
