There’s a big ole chunk of calculus on the distal of that molar. That would cause pain. Notice how the bone around it is darker; a sign of inflammation in the bone. I cannot speak for the lack of care that leads to root canals, crowns, extractions, implants etc. I’d say get a new dentist & hygienist
