Physicians are not “handcuffed” but must work within ethics, documentation, hospital policy, and the law.

For adults: Surrogates can decline care, even if physicians think it’s unwise, so long as it’s within the bounds of substituted judgment or best interest.
For children: Physicians do have an obligation to intervene if refusal constitutes neglect or serious risk of harm.
