I would say its all of that plus more - medical and social interferences as well. Even now with far less number of signers who are high proficient and deaf compared to past. Im keen to see what Wyatt add as well.

Did they mention the starting age of acquisition and type and duration of exposure to signed languages and models?

One of your avid followers here - please keep going as its critical situ here in Australia as I am seeing lot of adults with severe issues in life due to inaccessible language experiences. And it is harder when we try to explain to their families why...
