Marine engineer on the Great Lakes. Former tall ship sailor and military bomb tech. Mostly just here to lurk, but people keep following my account, so figured I should write something here...
You're either grossly incompetent, or complicit. Either way, you should resign. Almost makes me want to move to NY so I can help primary your sorry ass in 2028.

Sorry for the late reply...Starlink antenna got blown off by some nasty weather😆

There were absolutely mitigating factors, but I often work with the AB who was responsible for the anchor. The short version from him is: "A lot of stuff went wrong, but mostly, I just screwed up."

My main point is that it is absolutely possible for a large vessel to drag anchor for an extended period at speed without noticing.

When this happened, the ship was not in a restricted maneuvering area; that anchor should have been secured by a Devil's Claw, riding pawl, and windlass brake, same as any ocean ship with stockless anchors. Hate to say it about my ship, but the culprit here was just laziness and incompetence.

No real disagreement when your conclusion, but the ship I currently work on did exactly that a few years ago....NTSB incident report here
