If you have vision ins you might not need a referral at all (assuming you're in the US)!! Everyone should get their eyes checked yearly to screen for glaucs and cancer.

Can you have someone else write it for you? Like give them an outline with the job summary and your matching quals/accomplishments for them to fill in so you don't have to feel gross about tooting your horn. Bc someone else is tooting your horn.
