it is one of the few medications out there which are common enough and capable of causing oral birth control to fail, which is why i historically have cautioned british trans men about such, because many british people DIY it for ADHD.

it is a CYP2C19 inhibitor [see the two images] and an inducer of CYP1A2 / CYP3A4- so it'll reduce concentrations and length for oral estrogen, in example. but also like, dozens of medications in general.

is the milk safe? there's nothing really special to it- and it depends on the pharmacological agents present in the individual, to their milk-to-plasma ratio. cocaine can pass through breast milk, in example. this will harm a baby. domperidone, though? it's fine. pubmed.ncbi.nlm.nih.gov/37352416/

in example: a common way of inducing lactation in women is through a gut motility agent called domperidone. domperidone does not cross the blood-brain barrier, but antagonizes dopamine receptors in our gut, to increase gastric emptying. when the body detects this- it nevertheless raises prolactin.

especially important to mention w/ this in mind is that there's just really not really any case reports of adverse events involving this combo [and drug poisonings / accidents happen all of the time] that i can find, which is highly suspect, as there's always case reports for this kind of stuff.

it benefits several political purposes though; it perpetuates the belief that assault is done by strangers instead of by people we know and trust, AND it adds a substance with prominent use by gay men / queer people / minorities to a schedule, heavily criminalizing it under the former belief.

good example is GHB. GHB is common to see recreationally in MSM. all you hear w/ GHB though is that it's used to "spike" drinks / is used for date rape. this is an urban legend like fentanyl in weed / hasn't been validated in literature. most assault is acquaintance-related, and via alcohol.

part of harm reduction for me is communicating accurate information, basically- it doesn't help that poppers historically have been maligned in medicine due to stuff involving gay men's sexuality and HIV/AIDS, that ends up seeming more on the side of homophobic propaganda than scientific inquiry.

i'd be negligent if i were to claim something were safe when it was not. but, it's not dangerous. meanwhile, things like cocaine [and its metabolite, cocaethlyene,] which people do not hold to the same PSA, is dramatically worse for one's heart than it- but people are nevertheless okay, to averages.

yes, the increased risk observed is a confounder specific to this population and why specifically they're being prescribed these medications- for angina, and the underlying issues which causes angina. they are not suddenly dying as is claimed in the original tweet. it's important context.
