Mostly size, but as I said the flowers really aren't male or female, more male-ish or female-ish. They tend to drop the more male ones

I had a tree here that was 99.999% dead when we moved in, someone had planted it bare root some time before and it got neglected or gophers, Whatever, it was a stick. I started watering it, and eventually it came back to life very, very slowly and took a good 3 years to produce fruit.

One other thing, if you prune it - try and remove whole branches as opposed to an overall trimming, like a hedge trimmer would do- fruit tends to appear on newer growth, so that type of pruning may reduce the ability to produce the next year

They do take about 3 years to produce fruit. a lack of bees could be an issue

Or...Pomegranate flowers are weird - each flower has male and female parts, but on a spectrum. You may have a tree or variety that produces a lot of more male-ish flowers and thus no fruit.

If you are in CA, they have hit the end of the growing season and aren't going to get bigger. If you want bigger fruit, remove a good percent of young fruit next year. The trees themselves are exceedingly tough and resilient. Thru do pretty well even if generally neglected.
