MRI FT uses routine cine images- no extra sequences needed. Once early and late strain rates are calculated, we can identify and distinguish between stages of diastolic dysfunction, incorporating diastolic function assessment into our cardiac MRI study.

Why is this relevant? Because these measures of left ventricular and atrial remodeling have been shown to be predictors of adverse cardiovascular outcomes. Knowing how strain rates correlates with prognostic markers opens the door to a more robust use in clinical settings 🩺

Both early and late diastolic strain rates were inversely associated with LV remodeling and estimated left atrial pressure, even after adjusting for covariates such as age, sex, and hypertension. Late diastolic strain rates were also inversely correlated with left atrial remodeling 🫀

Cardiac remodeling was represented by left ventricular mass to end-diastolic volume ratio (LVM/V), LVM index, left atrial volume index, estimated left atrial pressure, and nonischemic late gadolinium enhancement (LGE) 🫀

Peak early diastolic strain rates significantly decreased with diastolic dysfunction, while peak late diastolic strain rates were higher with grade 1 diastolic dysfunction compared to normal function, but progressively decreased as severity of dysfunction increased.

In this new study by Yeo et al. participants, either asymptomatic or with a variety of cardiovascular conditions, were prospectively enrolled. Degree of diastolic dysfunction was assessed by echocardiography and strain evaluation was performed via cardiac MRI FT.

How diastolic strain rates change across a spectrum of grades of diastolic dysfunction and whether this change is associated with cardiac remodeling is unknown, until now! Let’s see..

BACKGROUND: MRI FT is an imaging technique that evaluates myocardial deformation on routinely acquired cine images. MRI FT allows measurement of strain and strain rates, which can be affected by the presence of underlying diastolic dysfunction.
