Very interesting. We have had a couple of babies recently with quite severe biventricular dysfunction. Always challenging to work through the findings and which medications you think will have the most benefit and cause the least harm.

We use standardised solutions in our NICU. They are on imprest so immediately available. Most of our extremely preterm infants get IVN in the 1st 2 hours, when lines are in. There are rules around how much you give to keep protein intakes appropriate. Cheaper than individualised solutions as well.

Very helpful. We are having this conversation now in our service about assessment and management. We’ve had an ad hoc approach over the past few years but are looking to be a bit more systematic. @shezza622.bsky.social

That's a good suggestion. I wish that there were more RCTs on the utility of diagnostic tests. We'd need to stir up some dissent to get people to stop automatically ordering it. 🤔
