In healthcare, we love checklists, protocols, systems.
But how much do we focus on making people feel safe to challenge, question, or raise concerns?
That’s also where real safety is.

Yesterday, during a lecture at uni, we discussed a tragic aviation case:
A co-pilot who knew they wouldn’t have enough fuel but didn’t challenge the captain’s decision.
They died in that crash.

A seasoned perfusionist once told me:
“The problem with surgery is that it’s not like aviation. In aviation, you do the checklist because your own life depends on it.”
Until we see safety as that important, we’ll struggle to make it part of our culture.

Very interesting image! I’m learning plain film CXR so pardon me if I’m asking a silly question but there is any reason why the XR was done AP and not PA? Thanks for your time in answering this :)
