I know EXACTLY how it works. I was a pricing Actuary for 30 years. A sick/aging population of high utilizers and provider consolodation(greed) is driving premiums. Industry wide 83% of all premiums go directly to the providers as claims payments. Our system is way too expensive and unsustainable.

I'm not defending premiums, but with not for profit insurers, it's the health of their pools and the providers who drive the premiums. The not for profits must cover claims, andmin, and are required by law to put money in reserve. If they make too much money, they are must give it back.

FWIW....BCBS of TX is a not for profit.
