Pleural effusions can occasionally appear to extend into the mid/upper zones if they are large or if the patient is supine (as in this example). The irregular shape of the effusion in the first post is what points towards a complex effusion and urgent need to exclude empyema

You may think it looks obvious but it was missed by the treating clinicians. The main abnormality of concern here is a loculated L sided pleural effusion with associated consolidation highly suspicious for empyema. Appearance on US confirms complex effusion and pleural fluid sample confirmed empyema

Speaking for our site, I don't think there was a lack of equipoise.
The UK black population is highly concentrated in certain geographic areas (esp London). With the exception of St George's, there were few hospitals in these areas that recruited significant numbers of patients

Great thread! Corollary to this is make sure you're not just confusing a dilated hepatic vein for the IVC. The key to this is to double check in the short axis if unsure
