Going by what I have seen done in game most go with the horns continuing to grow throughout the tiefling's life, much like the horns of livestock. I would think that much like those animals having the horns trimmed wouldn't hurt physically. Mentally and emotionally hurt if done non concentually.

You needed the rest. I used to be hard on myself but finally accepted that self care isn't being lazy or selfish. It is necessary for good health.
