What are your favorite “high verisimilitude” magic systems for RPGs? Vancian casting is a great for resource management, but can really take you out of the fiction.

I've posted multiple alternatives to Vancian casting on my blog, but I want to hear what other systems people have thought of!

Would you kickstart and OSR game whose main premise is a fully rebuilt magic system that is fully BX compatible?

Random NPCs can be tough. Use these tables w/ 3d6 to generate it:

Voice
1 Posh
2-3 Neutral
4 Foreign harsh
5 Poor
6 Foreign fancy

Speech oddity
1-2 None
3 Overused (like, um, etc)
4 Slow/fast
5 big words but wrong
6 Hand talker

I significantly prefer advantage/disadvantage over the +-4 suggested by B/X D&D, because of the excitement and tension it has, even though the statistical impact is nearly the same. Here's my explanation:
